§ 155.04 PERMITS; ADVERTISING SIGN STRUCTURES; FEES.
   (A)   (1)   No person shall erect, structurally alter or relocate any sign or other advertising structure as defined in this chapter, without first obtaining a permit from the City Clerk, and making payment of the fees required.
      (2)   Application for a permit shall be made upon blanks provided by the City Clerk, and shall contain, or have attached thereto, the following information:
         (a)   Name, address and telephone number of the applicant;
         (b)   Location of building, structure or lot the sign is to be attached or erected;
         (c)   Position of the sign or other advertising structure in relation to adjacent buildings or structures;
         (d)   Two sets of drawings of the plans, specifications and method of construction, and attachments to the building or the ground shall be distributed to appropriate departments for compliance;
         (e)   Copy of stress sheets and calculations showing that the structure is designed for dead load and wind pressure in any direction in the amount required by this code, when required by the City Clerk, or designated representative;
         (f)   Name of the person or entity erecting the structure; and
         (g)   Other information as the City Clerk, or designated representative, shall require to show full compliance with this chapter.
   (B)   Unless specifically provided in another section of this chapter, the base fee for permits shall be $25. If sign includes electric services, the fee for permit shall be $50.
   (C)   If the work authorized under the permit has not been completed within six months after date of issuance, the permit shall become null and void.
