If the application is approved, the permit shall be issued by the Clerk upon payment of the fees and deposit, and upon providing proof of insurance and proof of state licenses as prescribed below:
(A) Permit fee for each fireworks stand or display permit: $50;
(B) Deposit fee for each fireworks stand or display permit: $150, refundable upon certification by Code Enforcement Officer that site is clean within 15 days after July 6 closing of the stand or following the display;
(C) Proof of liability insurance in the amount of $300,000 with the city as an additional insured; and
(D) Proof of required state sales tax permit and all required state licenses for retail sales or display of fireworks.
