(A) Prior to the issuance of any business license as set forth in § 110.10 of this chapter, the applicant shall submit in writing, on a form prescribed by the city, an application for business license.
(B) The application for business license shall contain, but not be limited to, the following information, to wit:
(1) Name of business;
(2) Owner of business;
(3) Location of business;
(4) Owner of real estate where business is operated;
(5) Number of employees; and
(6) Sales tax number issued by the state’s Tax Commission.
(C) Prior to the issuance of the license the city staff shall determine that the business complies with all zoning ordinances of the city, together with all other code provisions and Building Code requirements adopted by reference in the code.
(D) A copy of the occupancy permit issued on the premises where the business is occupied shall be attached to the application for business license.
