(a) Application and Review.
(1) An individual wishing to apply for consideration of a reserved space may request an application from the City Manager. The information provided on the application will determine the initial eligibility (verification of Ohio Bureau of Motor Vehicles (BMV) recognized disability (ORC 4503.44)).
(2) Upon filing of the completed application and fee of twenty-five dollars, ($25.00) by the person with the disability or his or her representative, and staff verification of eligibility, a site survey will be completed. The site survey eligibility will be approved if:
A. No available off-street parking is accessible to the applicant.
B. The parking space requested is an existing on-street space, in front of or, in the case of a corner lot, alongside the applicant's residence.
1. The parking space shall not encroach upon a neighboring property's frontage unless written approval for the encroachment is secured from the resident(s) and owner of the property where the space will be located.
2. Requests for a parking space located other than in front of or alongside the applicant's residence will be considered on a case-by- case basis, and may be considered for approval only upon written concurrence from the resident(s) and owner of any affected property.
(b) Response and Installation.
(1) If the application is granted by the City Manager:
A. The Service Department will establish the location of the space, send a confirmation letter to the applicant with an invoice for the first fifty dollar ($50.00) installation fee installment, or the entire seventy-five dollar ($75.00) fee if the applicant is temporarily disabled.
B. The applicant remits the fee to the Finance Director, who notifies the Service Department. The Service Department will then release the work order to the Street Department and mail the permit card to be displayed in the rear deck of the applicant's vehicle.
C. The appropriate sign and markings identifying the space will be installed by the Street Department (the sign shall display the same number as issued on the permit card.), within ten working days of authorization.
D. Prior to the third quarter of the permit, the Service Department will invoice the permit holder for the second twenty-five dollar ($25.00) installation fee installment payment, with a copy of the invoice to the Finance Director.
E. All permits will expire on the first day of May. Application shall be made for renewal at least 30 working days before the expiration date. A renewal fee of ten dollars ($10.00) shall be submitted at that time.
(2) If the application is denied by the City Manager:
A. A letter stating the reason for denial and explanation of the appeal procedure will be sent to the applicant.
B. Appeals may be sent to the attention of the City Manager, detailing the reasons that exceptions in the policy should be made in this specific case.
C. The City Manager shall grant or deny the exception based on the information submitted. If the application is granted the procedure shall be the same as detailed in the preceding section. The City Manager's decision is final.
