As used in this chapter:
(a) "Advanced life support" means the provision of complex specialized life- sustaining services, often with equipment for radio telephone contact with a physician or other hospital personnel, including, without intending limitation, the administration of intravenous solutions, the use of anti-shock trousers, the establishment and maintenance of a patient's airway, the defibrillation of the heart, the relief of pneumothorax conditions and the provision of cardiac monitoring.
(1) “Advanced life support, level 1 (ALS1) emergency" shall have the same meaning as that contained in the Medicare National Fee Schedule.
(2) “Advanced life support, level 2 (ALS2) emergency" shall have the same meaning as that contained in the Medicare National Fee Schedule.
(b) “Assisted at the scene-no removal” means that services were requested by the patient or on the patient's behalf and were provided at the scene, but the patient refused to be removed to the hospital.
(c) "Basic life support (BLS)" means the provision of such basic services as control of bleeding, splinting of fractures, treatment of shock, child delivery, cardiopulmonary resuscitation and the like, involving no advanced life support (ALS) procedures.
(d) "Good intent - no removal" means the service was summoned by a third party and was not requested by the patient or necessarily dictated by the patient's condition.
(e) “Mileage” means the number of miles from the point of removal of the patient to the hospital.
(f) "Nonresident" means a person who maintains his or her principal place of residence outside the corporate limits of the City.
(g) "Resident" means a person who maintains his or her principal place of residence within the corporate limits of the City.
