§ 110.02 APPLICATION AND FEE.
   (A)   Applications for a permit under this chapter shall be made to the Clerk-Treasurer on forms prescribed and furnished by him or her.
   (B)   Completed applications shall be accompanied by an application fee as set forth in the Fee Schedule, Chapter 98.
