§ 114.02  APPLICATION.
   (A)   Application for a permit shall be made on forms provided by the Village Administrator/Clerk.
   (B)   The forms shall set forth the applicant’s name and address and the legal description of the premises for which the permit is sought, and shall be accompanied by an operation plan and a restoration plan.
