725.02 APPLICATION.
   Every applicant for a license as a driver of a taxicab shall make application to the Mayor on forms to be supplied by the Mayor, which forms shall direct applicants to furnish such information as shall be required by the rules set up by the Mayor. The application shall be sworn to by the applicant before filing.
   (a)    The application must contain a statement that the applicant is:
      (1)    A citizen of the United States;
      (2)    Able to speak, read and write the English language;
      (3)    Is over eighteen years of age; and
      (4)    Is not addicted to the use of alcohol or drugs.
   (b)    Such affidavit shall be accompanied by the certificates of two reputable residents of the Municipality to the effect:
      (1)    That they have known the applicant for more than one year;
      (2)    That they have read over his affidavit and from their own knowledge are satisfied that the statements contained in the affidavit are true.
   (c)    Applicant shall further furnish to the Mayor the following:
      (1)    Evidence that he holds an operator's license under the laws of the State and
      (2)   Two photographs of himself of a size designated by the Mayor, which photographs shall have been taken within thirty days next preceding the date of application.
