§ 114.02 APPLICATION FOR LICENSE.
   Applications for ambulance licenses hereunder shall be made upon such forms as may be prepared or prescribed by the Common Council of the city, and shall contain:
   (A)   The name and address of the applicant and of the owner of the ambulance;
   (B)   The location of the station or stations at which the ambulance will be based in the city;
   (C)   The trade or other name, if any, under which the applicant does business or proposes to do business;
   (D)   The training and experience of the applicant in the transportation and care of patients meeting all criteria required for licensing by the state;
   (E)   A description of each ambulance, including make, model, year of manufacture, motor and chassis number, current state license numbers, the length of time the ambulance has been in use, and the color scheme, insignia, name, monogram or other distinguishing characteristics to be used to designate applicant's ambulance;
   (F)   A description of the communication system to be used by the ambulance company;
   (G)   Staffing provisions and qualifications;
   (H)   A certification of liability insurance for each and every ambulance owned or operated by the applicant in an amount not less than $300,000 providing for the payment of damages for the injury or death of individuals in accidents or damage to personal property, with proper endorsement to hold the city harmless therefor;
   (I)   Such other information as may be deemed reasonably necessary to a fair determination of compliance with this chapter; and
   (J)   An accompanying license fee of $100 per ambulance to be operated in the city.
