(A) Applications for license under the provisions of this chapter shall be filed with the City Manager on such forms as the City Manager shall prescribe.
(B) At a minimum, such application shall contain:
(1) A statement of the name, address and telephone number of the applicant;
(2) A list of those stockholders who own 10% or more of any corporation making application or of all partners in a partnership;
(3) A description of the type and condition of the equipment to be used for collection, transportation and disposition;
(4) A statement that the method of disposition to be used is in agreement with national, state and local health standards; and
(4) Such other facts or information as may be required by the City Manager or contained in franchise contract specifications or agreements.
