§ 150.46 PERMIT DISCLOSURE.

   The application for the occupancy permit shall disclose the following information:

   (A)   Address of the premises;

   (B)   Name and address of the owner;

   (C)   Name of tenant;

   (D)   Number of rooms;

   (E)   Bathroom facilities;

   (F)   General condition of the premises; and

   (G)   The application shall also disclose and occupations of all occupants.
