(a) The City will allow and shall pay the burial expenses of indigent persons, as required by Ohio R.C. 9.15, in an amount up to $950.00. In addition, a stone or concrete marker shall be provided by the City at the burial site. The location of all burials shall be the indigent section of Glenwood Cemetery or Forest Hill Cemetery. If burial in another area of said Cemeteries is requested, any difference in cost shall be paid by the requester.
(b) All persons seeking reimbursement from the City for indigent burial expenses shall be required to supply the Finance Director with the necessary information, on a form to be furnished by the Finance Director, in order to assure compliance with this section and to substantiate permissible reimbursement. No reimbursement shall be made for burial expenses allowed until such form is completed and the information is furnished.
