§ 150.91 PROGRAM ADMINISTRATOR.
   (A)   The village shall appoint a program administrator to oversee the registration of vacant properties.
   (B)   The program administrator or his or her designee shall have the power to enforce the Vacant Property Registration Act and to accept the registration form submitted by an owner.
   (C)   The program administrator shall create a registration form which the owner shall submit to register a vacant property. The form may be in paper and electronic form. The form shall require the following information:
      (1)   The name, street address, mailing address, telephone number, and, if applicable, the facsimile number and email address of the property owner and his or her agent;
      (2)   The street address and parcel identification number of the vacant property;
      (3)   The transfer date of the instrument conveying the property to the owner; and
      (4)   The date on which the property became vacant.
   (D)   The program administrator shall give advance notice to property owners of properties which appear to have been vacant for 180 days.
