909.03 DEDICATION OF PRIVATE STREETS, ROADS AND Rights-of-way.
   (a)   The dedication of any private street, road or right-of-way shall be instituted upon the petition of the abutting land owners to Council requesting an ordinance to dedicate such street, road, or right-of-way.
   (b)   It shall be necessary to complete the following requirements before dedication of the street, road or right-of-way. A preliminary plan outlining the street, road, or right-of-way alignment and improvements necessary to conform to these regulations shall be submitted to the Planning Commission for approval.
   (c)   Upon approval of the preliminary plan, a land survey of the street, road, or right-of-way, showing the centerline of the right-of-way, the areas of the right-of-way to be dedicated, and the abutting owner's property lines and frontages shall be prepared and furnished to Council.
   (d)   A dedication plat or location plan and warranty deeds from all abutting land owners transferring title to right-of-way areas to the Municipality shall be furnished to the Municipality. Such instruments shall be properly executed ready for recording.
   (e)   Upon approval of the dedication plat or warranty deeds, detailed plans for the improvement of the area to be dedicated shall be prepared showing all existing and proposed underground utilities, drainage facilities, surface structures and other improvements. These plans shall include surface and underground profiles, topography, and cross-sections. The plans must be submitted to and approved by the Engineer and by Council.
   (f)   The street or road shall have a minimum surface improvement of a six inch depth compacted road gravel base twenty-two feet in width, graded to a design or profile acceptable to the Engineer. There shall be drainage facilities with driveway culverts installed which shall be a minimum of twelve inches in diameter.
   (g)   The water lines, sanitary sewers, and storm sewers shall be installed except when directed otherwise by Council.
