(a) All original applications for an electrical contractor registration shall be made to the City Engineer and/or the Director of Public Services, or their designate, upon forms furnished by the City of Munroe Falls. Such forms shall contain:
(1) The name of the applicant and every officer of the corporation, each partner of a partnership, or other responsible business associate.
(2) Business address.
(3) Residence.
(4) Present employer.
(5) Years of apprenticeship served.
(6) Years as journeyman wireman.
(7) Years as contractor indicating the City and State of work.
(8) Number of years experienced in business of installing electrical wiring devices in accordance with the rules and regulations set forth in the National Electrical Code.
(9) References.
(b) No person or entity shall knowingly make a false statement or representation in an application.
