(a) All original applications for a heating, ventilation and air conditioning license shall be made to the City Engineer and/or the Director of Public Services, or their designate, upon forms furnished by the City of Munroe Falls. Such forms shall contain:
(1) The name of the applicant and every officer of the corporation, each partner of a partnership, or other responsible business associate.
(2) The business address.
(3) Residence address.
(4) Telephone number.
(5) How long the applicant has been in business in Summit County or the counties contiguous to Summit.
(6) Registered elsewhere in Summit County.
(7) References.
(b) No person or entity shall knowingly make a false statement or representation in an application.
