(A) Any charitable organization may apply for a permit. The application for a permit shall be made on forms supplied by the Mayor. The forms shall require the applicant to indicate the name and nature of the organization, the date(s) and means of solicitation proposed, and such additional information as the Mayor determines is necessary to assure that the charitable organization will comply with this subchapter.
(B) A charitable organization may not apply for a permit within three months after the date of its last permit.
