§ 51.25 EXTENSION; QUALIFICATION; TESTING; INSPECTION.
   (A)   For the purpose of this section, the following definitions shall apply unless the context clearly indicates or requires a different meaning.
   APPLICANT. Person or persons, corporations or public entity causing the extension to be made.
   EXTENSION. Construction of new water pipe line (main) where there is no main in front of property and the main must be extended to serve a particular premise or area.
   (B)   (1)   The Utility shall perform a capacity review of the plans for the proposed extension and determine if the Utility has the capacity or pressure available to service the proposed extension. The Utility shall have the right to modify the proposed extension to increase capacity or to service areas beyond the proposed extension. The Utility may participate in the cost of such modifications as approved by the Water Utility Board. Construction of any extension shall not be started until final approval of the plans has been granted by the Utility and by the Indiana Department of Environmental Management (IDEM) if applicable.
      (2)   The Utility operations staff shall be notified two working days before construction of the water extension begins and they shall be renotified at least one working day before construction restarts after an interruption of work. All work is to conform with the approved plans plus adopted Utility regulations and construction standards plus rules and regulations of the Indiana Department of Environmental Management (IDEM).
   (C)   (1)   Construction of any extension of the water distribution system shall only be done by a contractor who has qualified by having a $10,000 bond on file with the Utility along with a current certification of insurance in amounts specified by the Utility. The bond filed shall be for the use and benefit of the Utility in making needed repairs and/or completing construction.
      (2)   The Utility may disqualify for cause any contractor who fails to perform in accordance with the plans and specifications, the rules and regulations of the state, or the rules and regulations of the Water Utility Board.
   (D)   (1)   Inspections during construction will be done by the Utility; however, these inspections will not relieve the contractor of his obligations to conform to applicable rules and regulations.
      (2)   After laying and backfilling the pipe extension sections, the contractor shall perform hydrostatic testing, flushing and disinfection conforming to the construction standards for potable water systems. The Utility shall be notified at least two days before any water line testing and shall witness these procedures. The expense of after hours inspections shall be paid by the applicant or contractor requesting the special inspection.
      (3)   The contractor shall furnish all labor, tools, and equipment necessary to make the tests and to perform any work incidental thereto; contractor shall, at his own expense, repair any damage to the pipe line and its appurtenances or to any structure resulting from the testing. If the section tested fails the test, it shall be repaired and retested by the contractor, until the required pressure is obtained.
      (4)   Upon completion, the applicant shall provide in writing the certified results of testing and one set of as-built drawings of the extension. The Utility will review the documents submitted and perform an inspection of the installation.
      (5)   A list (i.e. “punch list”) of required corrections will be compiled and submitted within two weeks to the applicant. The applicant shall respond within four weeks that the punch list items have been completed within a stated reasonable time. This procedure shall be repeated as necessary and the applicant shall be notified in writing when the system has passed final inspection. The applicant shall submit one set of reproducible mylar copies of the as-built drawings prior to the Utility notification of final approval.
   (E)   Prior to final approval, there shall be no flow from the existing system into the extension except water for flushing and testing.
   (F)   The Utility shall not restrict the issuance of a building permit for a building that will connect to a water line extension if the water line extension applicant has posted surety sufficient to complete the water line extension with the Utility and agrees to turn the water line extension over to the Utility.
   (G)   The applicant shall pay for all materials, tapping machines, equipment, and labor supplied by the Utility along with an inspection fee sufficient to cover costs of inspection and engineering review of the plans and installed work. An estimate of the Utility charges for the above listed items will be prepared by the Utility if requested in writing. The request must include a complete listing of items desired from the Utility and an estimate of the anticipated installation and testing time required.
