§ 114.03 APPLICATION FOR LICENSE.
   (A)   An individual may obtain a peddler's or transient merchant's license, or a temporary business license, authorizing the sale, trade or peddling of merchandise within the corporate limits of the city, or the conducting of a temporary business within the corporate limits.
   (B)   (1)   Application for license shall be made at the Clerk-Treasurer's Office, upon forms approved by the city.
      (2)   Such application shall be sworn to or affirmed, and filed with the City Clerk-Treasurer at least 72 hours prior to the time in which the license applied for shall become effective.
   (C)   The application herein shall contain the following information:
      (1)   The name, address or headquarters of the individual applying for the license.
      (2)   If the applicant is not a person, the name and address of the applicant's principal office or the manager.
      (3)   The name and address of the individual or individuals who will be in direct charge of conducting the local operations.
      (4)   The time within which the business activity will be operated, and the location of the business in the city.
      (5)   Any application for license must also be signed by the party upon whose property the temporary business or business activities shall be located.
      (6)   A statement regarding whether the individual has engaged in, or been held civilly liable for, any fraudulent transaction or been convicted of any crime involving moral turpitude.
