§ 113.03 REGISTRATION REQUIREMENTS.
   (A)   (1)   All contractors shall first register with the city by making application with the Building Commissioner.
      (2)   All information and documents given by the contractor must be true and accurate.
   (B)   The registration application shall include the following information and documents:
      (1)   Company or person's name, address and phone number;
      (2)   If a company, the owner's name, address and phone number;
      (3)   A list of the specific types of work in which the contractor shall engage in the year for which the application is being submitted;
      (4)   A list of all subcontractors and other employees or personnel that shall work with or under the contractor in the year for which application is being submitted;
      (5)   A state plumbing license number, if engaged as a plumbing contractor or as a plumber;
      (6)   Whether the contractor carries liability insurance and is bonded and, if so, copies of all documents showing proof of insurance and bonding; and
      (7)   Any and all documents or other information showing that the contractor has failed any test given by any approved testing facility where testing is required in the contractor's field of work.
   (C) A contractor does not have to be insured and bonded to be registered with the city.
