§ 96.03 PERSONNEL.
   (A)   Food handler certificate. All food handlers shall obtain and maintain a food handler certificate from a state and local approved source. The certificate earned in this course is valid for two years from the course completion date and must be available for inspection by regulatory authority. This certificate is required to work in a food service establishment.
   (B)   Health card. A health card will be issued to individuals who meet the criteria in division (B)(1) or (2) of this section. This card will be provided to individuals who are required to present a certificate verifying that a negative TB result or a statement of good health has been provided by clinic or medical facility. This card does not authorize individuals to work in food establishments. This health card will be valid for one year at which time a new health card may be obtained. The requirements for obtaining a health card will be the following:
      (1)   A statement from a physician or agent that the applicant has had a complete physical examination and appears to be in good health; or
      (2)   A statement from a physician or agent that the applicant has had a negative reaction to a PPD/Mantoux tuberculin test (TB test).
   (C)   Statement and fee. The applicant shall bring to the office of the regulatory authority a statement of the foregoing requirement and upon paying a fee of $2.00 will receive a health card valid for one year.
