The applicant must be the person who will own, control or operate the tax service. The registration form shall contain the following:
(A) Name and address of applicant, specifying, in the case of any unincorporated association, partnership or firm the names and addresses of each member thereof and the title or interest of each member;
(B) Business name under which the applicant does or proposes to do business;
(C) Description of past ten years business experience of the applicant and if applicant has had a transportation service permit or license revoked and under what circumstances;
(D) Location and address of place from which the applicant intends to operate;
(E) Number and description of vehicles the applicant proposes to use, including make, model, year, seating capacity, Texas license registration number and color scheme. As vehicles are added or removed, that information must be provided to the Chief of Police within 30 days of occurrence;
(F) Applicant shall provide a list of all drivers employed with address, date of birth and current Texas driver’s license number. The Chief of Police shall be notified in writing of any drivers who have been removed and all new drivers who have been newly employed, including name, address, date of birth and Texas driver’s license number;
(G) Documentary evidence from an insurance company authorized to do business in Texas indicating a willingness to provide liability insurance, as required in this chapter and by Texas state law;
(H) Description of service to be offered and rates to be charged;
(I) Such additional information as the city considers necessary for the enforcement of this chapter.
