§ 4-3-36 APPLICATION FOR LICENSE.
   (A)   Every person, firm, corporation, society, club or association desiring to operate, maintain or have operated or maintained such machine or game, or any other similar type of machine or game, above described for gain or profit, in any place of public resort, shall first make application to the City Clerk for license therefor, stating the full name and address of the applicant, or if more than one person is connected with the firm, corporation, society, club or association, the name of all such persons and the officers of such clubs and the addresses of said officers.
   (B)   The applicant shall also state the following information upon printed application forms to be supplied by the City Clerk:
      (1)   Where the said pinball or other similar device is to be used and maintained, including the street and number;
      (2)   The age of the applicant, together with his or her place of residence;
      (3)   Whether or not applicant or manager of said premises is a citizen of the United States and a resident of the city;
      (4)   The name and address of said owner if it is other than the applicant in said application and said application shall be signed by the owner, or some duly authorized agent and filed with the City Clerk; and
      (5)   Examination of said application may be made by the Mayor of the city to determine that the applicant is of good moral character and of the premises in which it is proposed that such machine, game or device be operated for gain or profit, is one of public resort and not habituated by criminals, felons, vagrants or persons having police records of crime or offenses involving moral turpitude, and that the applicant otherwise has kept within the terms hereof.
