§ 3-1-3 HEALTH AND WELFARE FUND.
   (A)   Establishment. There is hereby established a fund known as the Health and Welfare Fund.
   (B)   Purpose. The Health and Welfare Fund is established for the purpose of receiving contributions of moneys from donors for the purpose of paying the water, sewer, debt service and garbage charges imposed upon the residents of the city and users of such services, and who are experiencing economic distress or difficulties and are suffering a hardship because of such distress and difficulty.
   (C)   Budget. There shall be a separate budget and/or appropriation line item established for the said Health and Welfare Fund.
   (D)   Deposit of funds. All funds received from donors for the purpose set forth in this section shall be deposited in a separate account designated as the Health and Welfare Fund.
   (E)   Eligibility. Those residents or users who seek assistance in paying their water, sewer, debt service and garbage charges shall apply to the Mount Carroll Ministerial Association for a certification that such assistance meets the standards established by the Association. Upon certification, the Association shall notify the City Collector that the resident and user is qualified for financial assistance and the amount of such assistance.
   (F)   Transfer of funds. Upon the receipt of the certification from the Mount Carroll Ministerial Association, the City Collector shall transfer the authorized funds from the Health and Welfare Fund to the Water, Sewer, Debt Service and Garbage Fund and credit the user’s account in the amount of the funds certified and transferred as long as the balance in the Health and Welfare Fund is adequate for such transfer.
   (G)   Accounting. An accounting of all funds received by and transferred from the Health and Welfare Fund shall be made available to the Mount Carroll Ministerial Association at the end of each fiscal year and upon the receipt of the city’s annual audit.
