(A) There shall be two classes of building sewer permits as follows:
(1) Residential wastewater service; and
(2) Service to commercial or institutional establishments or industrial wastewater service.
(B) In either case, the owner or his or her agent shall make applications on a special form furnished by the village. The fee per connection shall be paid to the village at the time the application is filed pursuant to §§ 52.001 through 52.020, 52.035 through 52.040, 52.055 through 52.066 and 52.080 through 52.084 of this code of ordinances.
(C) The permit application shall be supplemented by any plans, specifications or other information considered pertinent in the judgment of the Superintendent. The industry, as a condition of permit authorization, shall provide information describing its wastewater constituents, characteristics and type of activity.
