Any disciplinary action taken by the Chief of Police against any member of the Department shall be reviewed by the Police Committee within ten days after a written request for review is submitted to him or her by the member of the Department against whom such disciplinary action is taken, provided that the written request for review is submitted within five days after notification of the disciplinary action is given to the disciplined member of the Department. Failure to submit a written request for review within five days after notification of disciplinary action shall be deemed to be a waiver by the disciplined member of the Department of any and all right of review.
