Any person applying for a permit to engage in the operation of a massage parlor or as a masseur, under the provisions of this article, shall first file with the Director of Finance a written application for such permit on a form to be provided stating the name and address of the applicant, the name and address of the massage parlor, the name and address of the person the applicant represents, the names and address of all the masseurs and masseuses employed in the massage parlor.
