§ 114.032 APPLICATION FOR LICENSE.
   (A)   Contents of application. Any person desiring a license shall file with the Council a verified application containing the name of the applicant desiring said license, whether such persons so applying are 21 years of age, the type of institution to be operated, the location thereof, the name of the persons in charge thereof and such other information pertinent thereto as the Council, by regulation, may require. Application on behalf of a corporation or association shall be made by any two officers thereof or by its managing agents.

   (B)   Requirements of applicant. Before license shall be issued, the person applying shall submit evidence satisfactory to the Council and the proper health official that the licensee is not less than 21 years of age and is of reputable and responsible character. In the event the applicant is an association or corporation, like evidence shall be submitted to the members thereof and the persons in charge. All applicants shall, in addition, submit satisfactory evidence of their ability to comply with the provisions of this subchapter and all rules, regulations and minimum standards adopted hereunder.
