§ 111.066 APPLICATION FOR LICENSE.
   An application for the license shall be made, in writing, to the City Administrator and shall state the full name and address of the applicant, the location where the amusement rides, carnival or circus is to be conducted, the owner of the premises and other information as shall be required by the City Administrator.
