Licenses shall be issued by the Building and Zoning Administrator upon the applicant's complying with the following registration requirements:
(a) Giving his name, present address and his permanent address.
(b) Giving a brief description of the nature of the business and the kinds of goods or property to be peddled, solicited or otherwise sold.
(c) Paying the license fee required by this chapter for such license.
(d) Giving the name and address of the licensed employer and exhibiting credentials showing he is an authorized employee or representative of such licensed employer, if the application is for an employee's license.
(e) Furnishing two photographs of the applicant, of appropriate size, one of which shall be attached at all times to the license, and the other delivered by the Building and Zoning Administrator to the Chief of Police for his files.
(f) For applicants seeking a license to operate as an itinerant vendor, giving the location at or from which such sales will take place, including documentation of the consent of the owner of the property so named for the applicant’s use of the property.
(g) For applicants seeking a license to operate as an itinerant vendor, giving the dates on which such sales will be conducted.
