The application for a user's permit shall be sworn to by the applicant and shall contain the following information:
A. Name and address of the applicant;
B. The applicant's date of birth;
C. Where applicant intends to permanently store the explosives intended for use, and the storage security measures provided at the storage and use sites;
D. The applicant's intended use for explosives purchased pursuant to any permit that may be issued to the applicant; and
E. All such additional information as may be prescribed by the council in determining whether the applicant is qualified pursuant to section 4-6-3-1 of this chapter to possess such permit. (Ord. 458, 10-19-1970)
