§ 95.01 PERMIT REQUIRED.
   (A)   It shall be unlawful for any person to hold a special event, sponsor a special event, or cause a special event to be held without first having obtained a special event permit from the city.
   (B)   A special event permit shall not be issued until a fully completed special event permit application and payment of fees are received by the city. The special event permit application must comply with ordinances and applicable state and federal laws.
   (C)   Special events permit applications must be submitted to the city no less than 15 days prior to the event.
   (D)   The special event permit application shall include the following information
      (1)   Name, address, and telephone number of the applicant and designated representatives.
      (2)   A description of the event and times it will be conducted.
      (3)   A detailed location description and route if applicable with staging areas and parking descriptions.
      (4)   The purpose of the event.
      (5)   If roads and/or sidewalks will be closed, the applicant will be required to explain how motorists, business owners, and residents will be notified in advance of the event.
      (6)   Expected number of participants attending the event.
      (7)   Number and types of vehicles or animals involved in the event.
      (8)   Location size and types of any canopies, awnings, food service booths, or other temporary structures shall be shown on a map.
      (9)   Description and location of any sound systems, amplification devices and electrical devices to be used during the event.
      (10)   Estimated number of attendees or spectators.
      (11)   Details regarding food service and whether alcohol will be sold during the event.
      (12)   Details regarding traffic control and security for the event.
      (13)   Proof of insurance.
      (14)   Clean up plan - where will trash be disposed of, how long to remove any temporary structures and temporary toilets.
      (15)   A description of potential issues or dangers and how the applicant intends to remedy the same.
      (16)   An indemnity agreement, specifically indemnifying the city from any and all claims caused directly or indirectly by the special event.
