Applications for purchases of cemetery spaces shall be made to the City Clerk. The application shall be under oath and shall, among other information, contain the following:
(A) The legal name and address of the applicant;
(B) The legal name and address of the person whom the space is being purchased for, if different from the person applying for the purchase of said space; and
(C) Satisfactory proof of residency qualifications for resident fees.
