Whenever any trustee of a land trust, or any beneficiary or beneficiaries of a land trust, makes application to the City of Monticello for approval of a subdivision under this code relating to the land which is the subject of the trust, the application shall identify each beneficiary of the land trust by name and address and define his or her interest therein as required by ILCS Chapter 765, Act 405. All these applications shall be verified by the applicant in his, her, or its capacity as trustee, or by the beneficiary as a beneficial owner of interest in the land trust. If the application is filed by a body politic or corporate entity, it shall be verified by a duly authorized officer of the body politic or other corporate entity for whom the application is made. These verifications shall be in the form of 1 of the examples cited in Appendix C attached to the ordinance establishing this chapter and hereby adopted by reference as if set out in full herein.
