§ 111.035 APPLICATION.
   (A)   An applicant for a license or permit from the Local Liquor Control Commissioner shall submit to the Commissioner an application, using a form provided by the Commissioner, which shall be in writing, under oath, signed by the applicant, and shall be accompanied by the license or permit fee applicable to the license or permit sought.
   (B)   The application shall include, without limitation, the following information and statements:
      (1)   The name, age, and address of the applicant in the case of an individual; in case of a co-partnership, the persons entitled to share in the profits thereof, and in the case of a corporation, the date and state of incorporation, the object for which it was organized, the names and addresses of the officers and directors, and the names and addresses of all stockholders and the percentage of stock owned by each;
      (2)   The citizenship of the applicant, his or her place of birth and, if a naturalized citizen, the time and place of his or her naturalization;
      (3)   The character of and the length of time the applicant has been in business, and the amount of goods, wares, and merchandise on hand at the time of application;
      (4)   The location and description of the premises which is to be operated under the license or permit;
      (5)   A statement whether applicant has made similar application for either license or permit on premises other than described in this application, and the disposition of the application;
      (6)   A statement whether applicant has ever been convicted of a felony;
      (7)   A statement whether a previous license or permit by any state or subdivision thereof or by the Federal Government has been suspended or revoked, and if so, the reason therefor; and
      (8)   A statement that he or she will not violate any of the laws, rules, or regulations of the State of Illinois, of the United States, or the City of Monticello, in the conduct of his or her business.
