(A) If the operator filing the application for a license under this chapter is a corporation, the application shall list the names and addresses of all officers and directors and any individual, corporation or other entity owning 25% or more of the issued and outstanding shares of every class of stock of such corporation.
(B) If the operator filing the application for a license under this chapter is a limited liability company, the application shall list the names and addresses of all members of the limited liability company.
(C) If the operator filing the application for a license is a partnership, the application shall list the names and addresses of all partners.
(D) If the operator filing the application for a license is a trust, the application shall list the names and addresses of all trustees and/or co-trustees.
(E) The listing required of any corporation, trust or partnership shall be repeated and further repeated for any corporation, partnership or other entity who or which appears as a shareholder, trustee, co-trustee, or partner on the application.
