(A) Each permit holder must complete a village Bi-Monthly Report Form and file it with the Clerk-Treasurer. The report form must be submitted by the 15th day of February, April, June, August, October and December. This report form will allow the village to comply with the reporting requirement of the Ohio Department of Natural Resources, Division of Litter Prevention and Recycling.
(B) Each permit holder must also complete a village Semi-Annual Report Municipal Solid Waste Form by the 15th day of July and the 15th day of January of the following year and file it with the Clerk-Treasurer. This report form will allow the village to comply with the reporting requirements of the Portage County Department of Health and/or the Summit County Solid Waste District.
