§ 150.201 INSTALLING OR REPLACING SIDEWALK WITHIN RIGHT-OF-WAY.
   (A)   Right-of-way permit application (to allow work within village R/W).
      (1)   Submit a right-of-way permit application to Village Clerk (with fees and deposit).
      (2)   Submit neat drawings of proposed work (see sample work plan).
      (3)   A zoning permit is not required.
      (4)   The Village Engineer reviews the zoning application.
      (5)   If the work includes the walk and drive apron, a single application is acceptable.
      (6)   If the work involves only the drive and/or apron, submit a driveway application.
   (B)   Village fees.
      (1)   Right-of-way permit application fee required.
      (2)   Deposits are required to cover direct costs to the village and provide money for the village to correct sub-standard or incomplete work. Refer to the latest Village Permits and Approvals Fee Schedule for application and deposit fees.
      (3)   Deposits may be increased by the village for complicated or larger projects.
      (4)   Unused deposits as determined by the village will be returned upon the village's approval of the work.
   (C)   Village standards.
      (1)   The contractor performing excavations within the village is required to contact OUPS, OGPUP, 811 or individual utilities as required to locate and protect underground facilities. Record drawings are not exact and care should be used when excavating near utilities shown on plans or marked in the field. The village should be contacted directly for the location of storm sewers.
      (2)   See the village standard roadway drawings for sidewalk construction requirements.
      (3)   It is recommended that property owners provide contractors the village standard details to obtain quotes for the work.
      (4)   Sidewalks shall not unreasonably direct water to a neighboring property.
      (5)   Sidewalks and curb ramps at intersections must meet ADA requirements.
      (6)   Sidewalks shall not trap or cause ponded water.
      (7)   Sidewalks shall be constructed to freely drain to reduce ice formation.
      (8)   Sidewalks shall not be installed to create trip hazards.
      (9)   Sidewalks through drives shall be thickened per the village details.
      (10)   Disturbed areas shall be repaired with a minimum of three inches of screened topsoil, a high quality residential seed mix, mulching and watering/fertilizer as needed to ensure full germination within 60 calendar days.
   (D)   Inspections.
      (1)   Provide the village 48 hour (two business days) notice of work to allow scheduling of inspection.
      (2)   Inspection by Zoning Inspector or Village Engineer.
      (3)   Pre-pour inspection of forms and base preparation at a minimum.
      (4)   Inspection during pour possible.
      (5)   Adding water to concrete mix to make it easier to work is prohibited.
      (6)   Apply a cure as soon as practical to prevent surface drying.
