§ 40.05 ANNUAL REPORTING AND FEE.
   MVP grant recipients shall provide evidence documenting payroll commitments. This information will be provided by submission of W-2s and employee income tax withholding information for each year covering the facility no later than March 31 of any calendar year in which a grant was provided. These forms will be reviewed by the Finance Committee at their first regularly scheduled meeting in April. The Finance Committee will make a recommendation to the Mayor and Council regarding the continuance or discontinuance of each MVP agreement and grant. An annual monitoring agreement fee of $250 will be charged any owner/business receiving benefits under this program. The annual monitoring fee will be used to offset costs associated with program administration.
