905.02  PERMIT REQUIREMENTS.
   The issuance of such permit is contingent upon the following requirements:
   (a)    A cash bond in an amount to be determined by the Village Administrator to insure the proper replacement of the pavement with a minimum of two hundred fifty dollars ($250.00) shall be posted with the Village Administrator.
   (b)   All necessary permits such as water, sewer, etc., shall be applied for and paid in full at the Office of the Water Department before the Village Administrator can issue a street breaking permit.
   (c)    It is mandatory that cuts or breaks in any improved street or alley be made with a pavement saw and done in a neat and orderly manner. No busting hammer shall be permitted for any primary cut in the street or alley, previously improved.
   (d)    Any repair, replacement or new installation shall be made in such a manner that it meets the inspection and approval of the various department heads, under whose jurisdiction the utility prevails.
   (e)   All such work shall be performed during the hours between 8:00 a.m. and 4:00 p.m. and no excavation shall remain unfilled for more than five days without the approval of the Administrator.
   (f)   All necessary safety devices, such as barricades, flashers, etc., shall be the responsibility of the person, firm or corporation acquiring the permit.
   (g)   Such bond shall be retained by the Village until such time as any necessary repairs and repaving are completed to the satisfaction of the Village Administrator. If at any time the Administrator shall determine that the work will not or has not been completed in compliance with this section the bond shall be declared forfeited.
