§ 24-504  AMUSEMENT PERMIT.
   1.   After the effective date of this Part, any person desiring to conduct, or to continue to conduct, an amusement within the Borough of Minersville, shall file in the Borough office an application for a permanent amusement permit, or a temporary amusement permit, as the case may be, and shall pay the fee for such permit required by this Section. In the case of any amusement that is to continue for longer than ten days, a permanent amusement permit shall be issued at a fee in an amount as established, from time to time, by resolution of the Borough Council. In the case of any amusement that is to continue for ten days or less, a temporary permit shall be issued at a fee in an amount as established, from time to time, by resolution of the Borough Council.    2.   The application for a license shall be made to the Borough upon suitable forms to be provided by the Borough, setting forth under oath:
      A.   The name of the Borough.
      B.   Whether a temporary or a permanent permit.
      C.   The name and address of the person receiving the permit.
      D.   The location of the amusement covered by the permit.
      E.   The type of amusement.
      F.   The period for which the permit is issued (permanent permits shall be good until December 31 of the year in which issued; temporary permits shall be good until the last day the amusement is conducted).
      G.   The number of the permit.
      H.   The date when the certificate is issued.
      I.   The signature of the issuing officer of the Borough. [A.O.]
   3.   No license shall be granted until the designated agent of the Borough, has inspected such place or structure in which the business is to be conducted and endorses upon the application that the same is suitable and safe, and will not be prejudicial to the public health and welfare. [A.O.]
   4.   Every permit shall be issued in duplicate. The original, to which the Borough seal shall be affixed, shall be given to the person applying for the permit, and the duplicate shall be kept on file in the Borough office. [A.O.]
   5.   In case of the loss, defacement or destruction of any permit, the person to whom the permit is issued shall apply to the Borough, which may issue a new permit, for which a fee in an amount as established, from time to time, by resolution of the Borough Council shall be charged. [A.O.]
