Any person, including an applicant for a therapeutic massage establishment permit, who engages in the practice of therapeutic massages as herein defined shall file an application for a therapeutic massage permit with the Chief of Police upon a form provided by the Chief of Police and shall pay a non-refundable filing fee of $25 for an original application and $25 for a renewal application, to the Village Treasurer, who shall issue a receipt which shall be attached to the application filed with the Chief of Police.
