§ 154.09 CERTIFICATE OF BUILDING MAINTENANCE AND OCCUPANCY APPLICATION.
   (A)   Application for a certificate of building maintenance and occupancy required by this code shall be made by the owner of the structure containing the commercial/industrial unit or by his or her duly authorized representative. If the application is made by a person other than the owner in fee, it shall be accompanied by a signed statement of the owner to the effect that the applicant is authorized by the owner in fee to make such application. The full names and addresses of the owner or the name and address of the responsible party if the owner is a corporation, trust or other type of legal entity that is not a natural person and the name and address of the applicant shall be stated in the application.
   (B)   The application for such certificate shall be submitted in such form as the Building Inspector/Code Administrator prescribes and shall be filed with the Building Inspector/Code Administrator whenever one of the events described in § 154.08 requires an inspection and issuance of an occupancy permit.    (C)   It shall be unlawful for any person to knowingly make any false statement on an application for an occupancy permit.
