(a) Applications for street openings, trenches, sidewalks and driveway aprons shall be made in writing to the City Engineer upon such forms as shall be provided by the City Engineer. Applications shall state the location of the proposed work, the name and address of the applicant and any other information deemed pertinent by the City Engineer. Any application for a trench located in the paved portion of a public right of way shall be approved by the City Engineer.
(b) Applications shall include a clause whereby the applicant agrees to indemnify and hold harmless the City from all loss or damage that may result from the making of such openings, trenches, sidewalks or driveway aprons. Such indemnity shall cover all loss or damage resulting from the restoration and maintenance of such work.
