§ 110.04 APPLICATION.
   Any peddler, solicitor, transient merchant, or itinerant merchant, as defined in § 110.01, requiring a permit to operate within the city shall make and file with the City Finance Officer a sworn application in writing on a form furnished by the City Finance Officer, which shall state the following information:
   (A)   Name and description of applicant;
   (B)   Address of applicant (legal and local);
   (C)   A brief description of the nature of the business and the goods to be sold or the services to be rendered; if goods, wares, or merchandise are to be sold, whether the items are manufactured by applicant; and in the case of farm or orchard products, whether these products are raised or produced by the applicant;
   (D)   If employed, the name and address of the employer, stating the exact relationship between employer and applicant;
   (E)   The number of days or length of time for which the permit is desired;
   (F)   If a vehicle is to be used, a description of the make and model together with the permit number or other means of identification; and
   (G)   A copy of applicant’s state sales tax permit issued by the state, if any.
