All applications for use permits shall be filed with the Village Administrator and shall include the following information:
(a) The identity of the permit applicant;
(b) A description of the services that will be offered or provided by the applicant through the proposed facilities;
(c) A description of the transmission medium that will be used to offer or provide such services;
(d) Preliminary engineering plans, specifications, drawings and maps of the facilities located, or to be located within the Village, all in sufficient detail to identify:
(1) The proposed locations and routes for applicant's facilities;
(2) The location of all existing overhead and underground facilities used to provide services in the public way along the proposed route;
(3) The specific trees and bushes, structures, improvements, facilities and any other obstructions that the applicant proposes to temporarily or permanently remove or relocate;
(e) If applicant is proposing to install facilities in existing ducts or conduits within the public ways, information in sufficient detail to identify the capacity available for the installation of additional facilities in such ducts or conduits before and after installation of applicant's facilities;
(f) If applicant is proposing to install new ducts or conduits within the public ways, information regarding the location, depth and capacity of such new ducts or conduits;
(g) A preliminary construction and operations schedule;
(h) Financial statements prepared in accordance with generally accepted accounting principles demonstrating the applicant's financial ability to construct, operate, maintain, relocate and remove the facilities;
(i) Information in sufficient detail to establish the applicant's technical qualifications, experience and expertise regarding its proposed facilities and service;
(j) Evidence that the applicant has obtained or will obtain all other governmental approvals and permits to construct and operate the proposed facilities and to provide the proposed services consistently with all applicable laws;
(k) The area or areas of the Village the applicant proposes to serve and a schedule for build-out to the entire area addressed by the permit;
(l) Such other and further information as may be required by the Village Administrator or the Village Engineer.
