All service providers shall register by February 1st of every year following the year in which the permit as required by Chapter 975 has been obtained with the Village Administrator pursuant to this Chapter 973. Any service provider who offers or provides service at the time of the effective date of this legislation shall register with the Village pursuant to this Chapter 973 within sixty (60) days of the effective date of this legislation. Registration shall include the following information:
(a) The identity, legal status and federal tax identification number of the registrant, including any affiliates.
(b) The name, address and telephone number of the local officer, agent or employee responsible for the accuracy of the registration statement.
(c) A description of registrant's existing or proposed services and facilities within the Village, including a current map or drawing, in a format specified by the Village, that clearly locates all existing and proposed areas of services or facilities.
(d) Evidence that the registrant has received all permits, licenses, certifications, or other approvals required by all applicable state and federal regulatory agencies, including the Public Utilities Commission of Ohio and the Federal Communications Commission, to provide services and locate facilities within the Village.
(e) Such other information as the Village may reasonably require.
