Applications for licenses and permits required by this title shall be made in writing to the village clerk upon forms provided by said clerk. Each application shall state the name of the applicant, the permit or license desired, the location to be used, if any, and the square footage of the premises to be used by the licensee, and each application shall contain such additional information as may be required by this title or requested by the village officials. No permit or license shall be issued or renewed until the applicant shall have furnished all information requested by the clerk or other municipal official and all other provisions of village ordinances have been complied with including, but not limited to, all applicable inspection fees, license fees, past due water bills, repair costs owed to the village of Midlothian, liens and citation fines having been paid to the village.
