§ 37.188 ABSENTEE REQUESTS.
   (A)   The Absentee Request (HR Form   ) is available to employees from the City Clerk and in the appendix to the Employee Handbook. When this form is submitted by the employee, the form will be sent directly to the employee’s immediate supervisor, who will be responsible for exercising managerial discretion in deciding whether to approve or deny the leave request.
   (B)   Employees are required to submit an Absentee Request (HR Form   ) whenever any type of leave is requested or taken as outlined in the policies contained in this chapter. The employee is required to provide the date and time of the leave. Employees are permitted to use any type of leave time in 15-minute increments.
   (C)   Whenever possible, employees are required to submit an Absentee Request Form before the leave is taken. If it is impossible or impractical for the employee to submit an Absentee Request Form prior to taking the leave, the employee shall submit the form immediately upon return to work. Any employee who takes leave time and fails to submit an Absentee Request Form shall be subject to disciplinary action.
