Every applicant filing an appeal shall state the reason for the appeal. If appertaining to real estate, a description of the property, with information as to the street number, if any, and also the assessor's plat and lot number. If appertaining to tangible personal property, the description and location thereof. The filing must be in writing notarized and properly signed by the applicant (if an individual) or by a corporate officer (if the applicant be a corporation).
